Proceed to 1911 Insurance located at the ground floor of St. John Hall and submit the accomplished Animal Related Incident Report, the original receipts of the consultation fee and vaccine charges, doctor’s medical certificate, vaccination card issued by the medical facility and photocopy of your DLSU ID.
