What are the reasons why my Agreement Form is not acceptable for submission?

The scanned copy of the Agreement Form that you submitted is not properly filled out.

The following are possible causes why Agreement Forms are not acceptable:
- wrong Academic Year and/or Term (or trimester) is indicated
- no Academic Year and/or Term (or Trimester) is indicated
- the applicant signed in the portion for the parent's signature
- the parent/legal guardian who signed on the portion for the parent's signature is not the parent/legal guardian on record

Please fill out the form correctly, then email a copy to admission.requirements@dlsu.edu.ph with the email subject Incomplete Requirement/s for Term {please specify the term} AY {please specify the AY} as soon as possible.
